How to Request Accommodations

Once appropriate documentation has been received and evaluated and accommodations have been approved, a Notification to Instructors form can be generated. The form lists all the student's approved accommodations. At the student's request, this form is used to communicate with faculty and staff about the accommodations needed for the student to have equal access. LOAs must be requested and issued on a term-by-term basis.

To receive accommodations, students should:

  1. Make an appointment to meet with his or her advisor prior to the start of term to discuss options and any questions or concerns about the requested accommodations
  2. Sign LOA form and give the form to appropriate professor/s (we recommend within the first two weeks of class)
  3. Discuss with professor/s about how the accommodations will be arranged (we recommend that students meet with their professors before or after class or during office hours)
  4. Inform OSD advisor if there are any issues in receiving accommodations

The student is responsible for picking up his/her LOA at the start of each term. If you do not give the LOA to your instructor, the instructor does not have to provide your accommodations. Accommodations listed on the Notification to Instructors form may change during the course of a student's enrollment due to changes in disability status, disability documentation or program standards and requirements.

Temporary Physical Impairments

Temporary accommodations may be provided as a service to students with temporary physical impairments so your enrollment can continue with minimal disruption. Temporary impairments are not covered by the Americans with Disabilities Act (ADA) because this law applies to only permanent disabilities.

OSD will require you to meet with an advisor and provide a letter from physician or medical documents showing the nature and extent of the injury in order to receive assistance with course tests.
