Department Moves

To ensure OIT is responsive to departmental moves, please submit the following form not less than 5 days prior the requested move.

Has Campus Facilities been contacted to ensure sufficient power outlets, movement of furniture and computer equipment to new site? Yes      No
This must be completed prior to OIT coming to the new site to setup/hookup PCs.
Does any machine have a static IP? Yes      No
If you are not sure check NO.
Do you have a Network Printer? Yes      No
This will require our Network personnel to ensure it functions at the new location.
Do phones and/or Faxes need relocated? Yes      No
If yes, please include extensions and/or fax numbers below.

Note:  
  1. Please ensure facilities personnel leave enough space between the furniture and power outlets/wallports.
  2. Please ensure there are a sufficient number of surge protectors in place.
  3. Directory Change Request Link: At the completion of the move each individual’s  directory needs to be updated.
