Daily Health Assessment
CDC Daily Health Assessment
Faculty, staff and students who are working or learning on-site are required to conduct a daily health assessment prior to reporting to campus.
Use this tool daily, prior to coming on campus, to help you make determinations about to campus to work or learn. Follow these simple steps to take the health assessment each day:
- Go to the website: Center for Disease Control and Prevention Testing.
- Click on the “Self-checker” icon. A “Coronavirus Self-Checker” window will open.
- Read the introductory information. After selecting “I agree,” the system will guide
you through a series of questions in a virtual chat format.
- Once you complete the questionnaire, you will receive advisement on whether to seek medical attention, get tested or a message that “No COVID-19 testing is needed at this time.”
If you receive advisement to seek medical attention or get tested, please stay home, notify your supervisor and seek appropriate medical attention to get tested. Follow the protocols for Illness Reporting.
After completing the assessment, if the message indicates “No COVID-19 testing is needed at this time,” you are cleared to come on campus.
Reporting
If you feel ill, test positive for COVID-19 or have been in contact with someone who tested positive, contact Tanya Mahan, COVID-19 case manager at COVIDillness@valenciacollege.edu or 407-299-5000, extension 3047.
