Under the state benefits, both the PPO and HMO health plans have a standard option and a high deductible option. When you enroll, you will choose between either the PPO or the HMO benefit, and then you will elect either the standard or high deductible plan.
The HMO insurance carrier options are segregated by region. Most employees will have a choice between four different health plan options – two Florida Blue PPO plans and two HMO plan options. But if your home address and work address fall into two separate HMO regions, you will have the choice between six different health plan options.
2025 Benefits Open Enrollment Dates
Monday, October 14 – Friday, November 1, 2024
For the health insurance benefit, there are only two tiers – employee only coverage or family coverage, so whether you have one dependent or more, you will have family coverage.

All employees will be eligible to enroll in the two Florida Blue PPO plan options.

Search OptumRx, the State Group Insurance Program’s Prescription Benefits Manager for the state’s PPO and HMO Plans. Find a network pharmacy and view medication pricing and coverage under each of the health plan options.
HMO Plans
HMO plans are provided by Aetna, United Healthcare, and Capital Health Plan. Employees will be able to enroll in the HMO plans available their region.
