Letter of Accommodation Requests

To receive your Letter(s) of Accommodation each semester, the student must complete the following LOA Request.

Students must be fully registered with the Office for Students with Disabilities in order to complete the Letter of Accommodation Request. Request must be completed for every semester the student is seeking academic accommodations.

  1. Access the Link
    Click or type this link into your browser to access the online registration form: Letter of Accommodation Request
  1. Complete the Student Information Form
    1. Fill out the form with the following information: Once the student has accessed the link, they will need to complete the student information form.
      1. Name, VID, Date of Birth, Phone number and Valencia College email address
      2. Select the Preferred Campus
      3. Answer the Required Questions
      4. List all the courses you would like to receive accommodations in
      5. Add any additional information regarding accommodations, if necessary
      6. Complete Acknowledgement statement
